On or about 12 December 2022, employees of D and S Window Manufacturing, Inc. dba. Rivertown Windows located at 9 Trautman Court in Sainte Genevieve, Missouri were exposed to hazards relating to powered industrial trucks. Rivertown Windows had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.

Rivertown Windows located at 9 Trautman Court in Sainte Genevieve, Missouri were exposed to point of operation hazards while operating a Delta Unisaw 10" Tilting Arbor Saw. The saw in the production area did not have a guard in place.
