29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about December 20, 2022, employees working on and around the Enco Metal Cutting Band Saw (Model #137-3175 and Serial #271 0596) were exposed to laceration and amputation hazard, in-that the employer did not ensure the unused portion of the blade was guarded. The guarding for the unused portion of the blade had been wired/tied up making it un-operational.

29 CFR 1910.303(g)(2)(i): Except as�elsewhere required�or permitted�by this�standard, live�parts of�electric equipment�operating at�50 volts�or more�shall be�guarded against�accidental contact by�use of�approved cabinets�or other�forms of�approved enclosures�or by�any of�the following�means. (a) On or about December 20, 2023, employees working on Tesker Thread Rolling Machine (#630-2,) were exposed to an electrical shock hazard. The energized wires were not properly guarded, in-that the energized wires were only guarded with wire nuts.
