29 CFR 1926.651(g)(1)(i): Where oxygen deficiency (atmosphere containing less than 19.5 percent oxygen) or a hazardous atmosphere existed or could reasonably be expected to exist, the atmospheres in the excavation were not tested before employees entered excavations greater than 4 feet (1.22m) in depth. Location: 518 Trapelo Road, Waltham, MA On or about 12/20/22, a supervisor was spotted working inside an 8-foot deep trench alongside a high-traffic roadway without having conducted atmospheric testing of the excavation prior to entering the space.
