29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Building located at 10160 West Florissant, Missouri: On or about December 16, 2023, an employee conducting roofing activity on a low-slope roof of a commercial, strip mall building was not provided or required to wear fall protection. On the date above, the worker fell through a hole where the existing decking was removed for repair.
