At 409 Parkway Dr, Park Hills, MO 63601, On December 20, 2022, Employees were exposed to the hazards associated with the unexpected energization of equipment associated with a Hesta Blasformtechnik Machine D - 73037 Type HV 230H S/N 201610706 which had posted Lock Out Tag Out instructions did not include specific requirements for testing the machine to determine and verify the effectiveness of lock out devices, tag out devices or other energy control measures.
