29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 19, 2022, during the operation of the Threading Line #2 (End of conveyer #14 and the entry to conveyer #15) at the work location 2669 Martin Luther King Jr. Blvd, Youngstown, OH 44510 (Threading Line #2,Threading Department) the employer did not ensure that employees were protected from struck by and caught-between hazards when correcting skewed steel pipe. Existing guarding did not prevent employees from reaching into the hazardous area, exposing them to injury at locations including, but not limited to the pipe stop.
