29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about December 20, 2022, during the installation of roofing material at a job site located at 3492 Ravenna Ave NE, Louisville Ohio 44641; during the installation of framing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 12 feet.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about December 20, 2022, during the installation of framing material at a job site located at 3492 Ravenna Ave NE, Louisville Ohio 44641; the employer did not ensure employees working on a roof of a single-family home were protected from a fall of approximately 12 feet by utilizing a fall protection system.
