29 CFR 1926.152(i)(6): In locations where flammable vapors were or could have been present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition: (a) On December 20, 2022, at or near 1580 48th Street in Grand Forks, North Dakota, employees were exposed to fire and explosion hazards while using a battery-operated reciprocating saw that was not rated for use in hazardous areas to install level sensor systems on three tanks containing flammable liquids.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) On December 20, 2022, at or near 1580 48th Street in Grand Forks, North Dakota, employees who were working with and/or around Styrene while installing level sensors on tanks containing Styrene were not provided effective training prior to the initial work assignment.
