29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:( (a) On or about December 20, 2022, employee(s) performing maintenance to the carrousel saw were exposed to hazards associated with energy control in that a hazardous energy control program had not been developed and it was not always rendered inoperative during maintenance.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about December 13, 2022, a carousel saw was being operated with its point of operation unguarded in that an employee assisting the operator to get a pre-cut bun of foam to go through the saw was exposed to a cut-by hazard when he climbed on top of the bun as it was being cut.
