29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the manufacturing facility located at 1119 N Jefferson St, Kearney MO, employees were exposed to rotating parts and nip points when one or more methods of machine guarding were not provided on a planer located in the extruder area. This resulted in hospitalization and amputation. This was documented on or about 12-15-2022.

29 CFR 1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: At the manufacturing facility located at 1119 N Jefferson St, Kearney MO, employees were exposed to sprocket wheels, chains and nip points when machine guarding was not provided on a planer located in the extruder area. The manufacture's side panels that guard the sprockets had been removed and replaced with cardboard. This was documented on or about 12-21-2022.
