29 CFR 1910.28(b)(3)(i)(A): The employer did not ensure that each employee on a walking-working surface was protected from falling into any hole or pit that was 4 feet (1.2 m) or more above a lower level by a cover or covers: On or about 12/13/2022, at the jobsite located at 100 Commercial Boulevard, Lauderdale by the Sea, FL 33308, an employee fell approximately 8 foot 5 inches to a lower level through an uncovered opening, 24 inches by 18 inches approximately, when walking out of a refrigerator cooler.
