29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Roof top of 2 Pine St Providence, RI 02903 :  On or about December 22, 2022, eye protective equipment was not used when an employee was using a powered hand drill while installing chimney caps.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Roof top of 2 Pine St Providence, RI 02903:  On or about December 22, 2022, employees were not protected from falling while working from an elevated work surface, approximately 75 feet above a lower level, installing a chimney cap.
