29 CFR 1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.  (a)  On or about December 20, 2022, the employer did not ensure that the canning production area was maintained in a dry condition.  Water was leaking from the canning machine onto the floor, thus exposing employees to hazards associated with slipping and falling.

29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface.  (a)  On or about December 20, 2022, the employer did not ensure that all required safety devices located at the de-palletizer operator area were in working order, in that the mid-rail chain adjacent to the pallet of can separation sheets was not secured and the midrail for the de-palletizer elevator had been removed.  This condition exposed employees to a fall when working at heights of 96 inches while working at the de-palletizer station.

This condition exposed employees to hazards associated with exposure to 3 phase, 120 volts AC electricity.   (b) On or about December 20, 2022, the employer did not ensure covers were installed over five open slots in the 2C circuit breaker panel.  This condition exposed employees to hazards associated with exposure to 3 phase, 220 volts AC electricity.
