Southside Of Building 147: On or about February 15, 2023, and at times prior thereto, employees were exposed to a struck-by hazard while installing concrete forms in an excavation with a spoil pile located less than 2 feet from the excavation wall. Northside of Building 147: On or about February 15, 2023, and at times prior thereto, employees were exposed to a struck-by hazard while installing concrete forms in an excavation with a spoil pile located less than 2 feet from the excavation walls.
