The Yale sit down forklift 938F, Model glc050lxneae087, Serial #B967V02899T, did not have functioning brakes. b) Warehouse- On December 16, 2022, the employer did not remove powered industrial vehicles from service when damage or unsafe conditions were identified. The Yale sit down forklift 940F, Yale sit down forklift 901B, and Yale sit down forklift 833, did not have functioning reverse lights. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET). Faro Cargo Handling Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.178(p)(1), which was contained in OSHA inspection number 1457881, citation number 1, item number 2 and was affirmed as a final order on April 30, 2022, with respect to a workplace located at Faro Cargo Handling Solutions, LLC, 630 Cargo Access Road, Chicago, IL 60666.
