(a)Location - 12 Putnam Street Winthrop, MA; On or about 12/22/2022, John V. Lozzi, as a controlling employer did not ensure that frequent and regular inspections of the job site were performed by a competent person. As a result, employees of subcontractor T2 Construction, Corp, were exposed to fall hazards of greater than 6 feet above a lower level.

(a) Location - 12 Putnam Street, Winthrop, MA; On or about 12/22/2022, employees were working at heights greater than 6 feet without personal fall protection. John V. Lozzi was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1310451, citation number 02, item number 01 and was affirmed as a final order on 08/06/2018, with respect to a workplace located at 1064 N. Shore Rd., Revere, MA.
