29 CFR 1910.269(l)(3)(iii)(A): (Minimum approach distances.)(The employer shall ensure that no employee approaches or takes any conductive object closer to exposed energized parts than the employer's established minimum approach distance, unless:) The employee is insulated from the energized part (rubber insulating gloves or rubber insulating gloves and sleeves worn in accordance with paragraph (l)(4) of this section constitutes insulation of the employee from the energized part upon which the employee is working provided that the employee has control of the part in a manner sufficient to prevent exposure to uninsulated portions of the employee's body). (a) On Dec. 23, 2022, employees were exposed to electric shock while making line repairs in that the employer did not ensure rubber insulating gloves and sleeves were worn when handling an energized 7,200 volt overhead powerline.

29 CFR 1910.269(m)(3)(ii): (Deenergizing lines and equipment) The employer shall ensure the installation of protective grounds as required by paragraph (n) of this section.The employer shall ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized are open. The employer shall render such means inoperable, unless its design does not so permit, and then ensure that such means are tagged to indicate that employees are at work. (a) On Dec. 23, 2022, employees were exposed to electric shock while repairing a downed overhead powerline in that the employer did not ensure disconnect switches supplying 7,200 volts were rendered open and tagged prior to employees making line repairs.

The employer shall ensure that, unless a previously installed ground is present, employees test lines and equipment and verify the absence of nominal voltage before employees install any ground on those lines or that equipment. (a) On Dec. 23, 2022, employees were exposed to electric shock while repairing a downed overhead powerline in that the employer did not test the lines and equipment to verify the absence of voltage prior to performing the subsequent step of installing grounds on those lines or equipment.
