29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) 876 North Lenola Road, Moorestown, NJ: The employer did not establish a program which included procedures, training, and inspections related to lock/out tag out to ensure isolation of energy sources when working on equipment such as but not limited to a high rail platform truck.  An employee was performing maintenance of a high-rail platform truck when the hydraulic cylinder failed causing the raised platform to come down on the employee causing crushing injuries when the platform fell trapped employee underneath.  Occurred on or about 12/27/2022.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
