Where the employer is supposed to put his/her signature on the Summary Form, the date next to it reads 1/26/2023 for the years 2019, 2020, and 2021.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) 4680 Table Mountain Drive, Golden, CO 80403: On or about December 28, 2022, and at times prior, employees were exposed to struck by hazards while working in the shop with forklift operators that were not certified by the employer.
