29 CFR  1904.32(a)(3):The employer did not certify an OSHA 300A Form or equivalent.  a) 4680 Table Mountain Drive Suite 150, Golden, CO 80403: On or about December 28, 2022, the employer failed to certify the annual summary of work-related injuries and illnesses. Where the employer is supposed to put his/her signature on the Summary Form, the date next to it reads 1/26/2023 for the years 2019, 2020, and 2021.

29 CFR  1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a) 4680 Table Mountain Drive, Golden, CO 80403: On or about December 28, 2022, and at times prior, employees were exposed to struck by hazards while working in the shop with forklift operators that were not certified by the employer.
