29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   a) Facility- On or about December 29, 2022, all employees who perform lockout/tagout required work on screeners, roller mills, jaw crushers, cone mills, and pvc piping were not provided with initial training prior to starting service or maintenance activities.

29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.   a) Facility- On or about December 29, 2022, and times prior to, employees operating forklifts in the facility did not receive formal instruction prior to operating the machinery.

29 CFR  1910.95(g)(6):At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  a) Facility: On or about December 29, 2022, employees exposed to noise at or above the action level of 85 dBA as an 8 hour time weighted average were not provided with an annual audiogram. The last audiogram provided to employees was conducted on or about January 8, 2019.
