Hollis, NY Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and materials from other employees who worked above. Protective helmets were not worn.

Hollis, NY Employees working on an unprotected second floor edge were exposed to the hazard of falling approximately 20 ft. to the ground.
