29 CFR  1910.22(a)(1): Places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:   a. Reliance Precast Systems, Inc., On December 30, 2022  the employer did not ensure the walking/working area in the production area were free from tripping and slipping hazards, in that materials, equipment and electrical cords were not kept in an orderly manner and  wood dust had accumulated in work areas. This condition exposed employees to slip and trip hazards.

29 CFR  1910.213(a)(12): For all circular saws where the conditions are such that there is a possibility of contact with the portion of the saw either beneath or behind the table, that portion of the saw was not covered with an exhaust hood, or, if no exhaust system is required, with a guard that was arranged as to prevent accidental contact with the saw:  a. Reliance Precast Systems, Inc., On December 30, 2022 the employer did not protect employees from injury, in that employees are working in the wood shop around the Delta Unisaw and Delta Contractor Table Saw, that were not guarded, with exposed blades. This condition exposed employees to amputations and lacerations.  Reliance Precast Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.213(c)(1), which was contained in OSHA inspection number 1371069, citation number 1, item number 2a and was affirmed as a final order on February 21, 2019, with respect to a workplace located at 4558 Grand View Blvd., Dacono, CO 80514.

This condition exposed employees to hazards associated with ingoing nip points.   Reliance Precast Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.213(c)(1), which was contained in OSHA inspection number 1371069, citation number 1, item number 4 and was affirmed as a final order on February 21, 2019, with respect to a workplace located at 4558 Grand View Blvd., Dacono, CO 80514.
