29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets: a. ION Solar LLC. at 3398 Hackberry Lane Johnstown, CO 80534: On December 30, 2022, the employer did not ensure employees working on the ground were protected from injury by falling objects by requiring employees to wear hard hats while solar panels were being installed. This condition exposed employees to head injury from falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels is not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a. ION Solar LLC. is failing to ensure that its employees are protected from fall hazards while installing solar panels at heights greater than 6 feet above a lower level. This practice led to four employees being unprotected from fall hazards on December 30, 2022, during installation operations on a two story single-family residential building on a roof that was subject to snowy/icy conditions without fall protection at 3398 Hackberry Lane Johnstown, CO 80534.
