29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:  a) Side Emergency Exit Doors - On or about 12/29/2022, employees were potentially exposed to fire hazards in that two emergency exit doors leading to the outside did not display an illuminated "EXIT" sign above it. The locked doors only had a printed red and white exit sign attached on the door.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Production Office / Production Floor - On or about 12/29/2022, the employer did not ensure that employees who worked with chemicals that have a high or low pH such as, but not limited to Sysco Classic Germicidal Ultra Bleach (pH=12.0-12.5), 30030 Degreaser (pH=13.7-14.0), 3023 Dish Machine (pH=13.3-14.0), 35400 Drains (pH=3.0-5.9), and 35401 Drains (pH=10.0-14.0) did not have the appropriate working facilities to use to quickly flush the eye(s) as well as a safety shower.

29 CFR 1910.178(l): Operators were not trained in the safe operation of powered industrial trucks:  a) Facility-wide: On or about 12/29/2022 the employer exposed employees to struck-by hazards in that powered industrial truck operators were not adequately trained and evaluated before operating the Crown electric stand-up powered industrial vehicle (Model: 55F-55-A062 RC3000, S/N: 1AF293821 and Komatsu electric  sit-down powered industrial vehicle (Model: FBI5M-2, S/N: 99234) to transport pallets of materials (equipment, product, etc.).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Chemical Room; Production/ Processing Area: On or before 12/29/2022 the employer exposed employees to chemical hazards in that, employees were not trained on the hazardous chemicals utilized in the workplace. Chemicals used at the site included but were not limited to Sysco Classic Germicidal Ultra Bleach (pH=12.0-12.5).
