The steel spuds were not made of nonsparking material.

29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators shall receive initial training in truck related tops including fork and attachment adaptation, operation, and use limitations: On or about January 24, 2023, at a facility located at 333 West 29th Avenue in Oshkosh, WI; the employer did not provide powered industrial truck operators with training on attachment use limitations. Forklift operator employees did not receive training on the use limitations of returnable packaging (metal reusable pallets) used for truck cabs. Limitations of the attachments required that no more than 14 inches of the forks could be exposed between the attachment and the mast.

29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty: At a facility located at 333 West 29th Avenue in Oshkosh, WI; employees at SST station 1 placed their arms, head and upper torso under elevated forklift loads during loading/unloading of truck cabs to and from load bars. While unloading cabs from the load bars, employees had to reach under the hanging cab to connect it to the returnable packaging (metal reusable pallet) forklift attachment. Employees also had to reach under the cab to disconnect the cab from the returnable packaging when loading the cab to the load bar.
