29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): a) Dock 1 - On or about September 8, 2022, the employer failed to ensure when an employee used an Abell-Howe 2-ton hoist rigged with polyester sling(s) to lift a bundle of 20 foot long steel bars, the employee kept clear of the suspended load. While the load was suspended, the employee was exposed to struck-by and crush hazards when the bundle of steel bars shifted and made contact with the employee's left hand that was on the load.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a) Dock 1 - On or about January 4, 2023, the employer failed to ensure compressed-gas cylinders containing oxygen were separated a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour from other compressed gas cylinders containing flammable gasses, including those containing a mixture of hydrogen and nitrogen, and propane fuel tanks. Affected employees were thereby, exposed to fire and smoke inhalation hazards.
