29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: Employees engaged in hanging Christmas lights at the work site located at West Edgewood Drive Jefferson, Missouri 65109 were exposed to fall hazards in that the employer did not ensure that employees working from aerial lifts were wearing personal fall arrest systems. Instances include but are not limited to: Instance 1. An employee was using a Genie S-45 aerial lift at the east end of the parking lot without any fall protection. Instance 2. An employee was using a Genie S-45 aerial lift at the west end of the parking lot without any fall protection.
