29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: Warehouse: On, or about, January 4, 2023, a Vestil telescoping boom was used with the following forklifts and was not listed as an attachment on the identification plate of each forklift: A. Toyota 8FG35U, serial number 5375132. B. Toyota 8FGU 32, serial number 63313.
