29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) Loading Dock: On or about January 4, 2023, employees were exposed to a fall in excess of four (4) feet from the loading dock bays to the lower surface outside the building due to a lack of fall protection such as a guardrail system, safety net system, or personal fall protection system.  Abatement certification required.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Shipping Area: On or about January 4, 2023, employees were exposed to electrical shock and fire hazards due to relocatable power taps being daisy chained to other relocatable power taps or extension cords while being permanently secured to the desk supports.  b) CIS Area: On or about January 4, 2023, employees were exposed to electrical shock and fire hazards due to relocatable power taps being daisy chained to other relocatable power taps or extension cords.  c) Parcel Area: On or about January 4, 2023, employees were exposed to electrical shock and fire hazards due to relocatable power taps being attached to the building structure such as desk supports.  Note: Under the United Laboratories 2015 White Book the section for Relocatable Power Taps (XBYS) states: "Relocatable power taps are not intended to be permanently secured to building structures, tables, work benches or similar structures, nor are they intended to be used as a substitute for fixed wiring." AND "Relocatable power taps are intended to be directly connected to a permanently installed branch-circuit receptacle outlet. Relocatable power taps are not intended to be series connected (daisy chained) to other relocatable power taps or to extension cords."  Abatement certification required.
