OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to being caught between the coil and the c-hook attachment.   a) On December 29, 2022, the employer did not ensure that qualifications for operators of the below the hook lifting device for the 1257 Mill Splicer JIB Crane, were properly using a C-Hook to lift a 5 ton split coil, were free from caught-between hazards in that employees were exposed to being pinched between the below the hook device handle and a stationary coil that was not properly centered. The operators were utilizing one hand to hold the wireless control for the JIB crane and the other hand to hold the handle for the c-hook to center under the load located next to additional stationary split coils, exposing the operators to being caught-between the stationary coils and the c-hook handle.  b) On December 29, 2022, the below the hook attachment, c-hook, did not have the required rated load making and identification markings for the c-hook lifting coils rated for approximately 10,000 pounds.  c) On December 29, 2022, the below the hook attachment, c-hook, alterations of the handle being removed and three welded tubes added on were done without the inspection being conducted by a qualified person.    a) Amongst feasible abatement methods ASME B30.20a 1996 Addenda Below-The-Hook- Lifting Devices section 20-1.6.2 Qualifications: Qualifications for operators of the below-the-hook lifting devices are as follows: (a) The operator shall be instructed in the use of the device by a designated person.  Instructions should include, but not limited to, the following: (1) application of the lifter to the load and adjustments, if any, that adapt the lifter to carious sizes or kinds of loads; (2) any special operations or precautions; (3) condition of the load itself required for operation of the lister, such as balance, degree of order of stacked loads, surface cleanliness, bending, load thickness, etc.    b) Amongst feasible abatement methods ASME B30.20a 1996 Addenda Below-The-Hook- Lifting Devices section 20-1.2.1 Markings: (a) Rated Load Marking: The rated load of the lifting device shall be marked on the main structure where it is visible.  If the lifting device is made up of several lifters, each detachable from the group, these lifters shall also be marked with their individual  rated loads.  (b) Identification: a nameplate or other permanent marking shall be affixed displaying the following information: (1) manufacturer's name and address (2) serial number (3) lifter weight, if over 100lb (4) rated load.  c) Amongst feasible abatement methods ASME B30.20a 1996 Addenda Below-The-Hook- Lifting Devices section 20-1.3.1 Inspection Classification: (2) Altered, repaired or modified lifting devices shall be inspected by a designated person.  The inspection may be limited to the provisions affected by the alteration, repair or modification, as determined by a qualified person.
