29 CFR 1910.178(l)(4)(i): The powered industrial truck (PITs) operators were not trained and evaluated at least once every three years: a. On or about January 5, 2023, at 9855 Mining Dr., Suite 102, Jacksonville, FL, the employer exposed employees to a struck-by hazard, in that, forklift operators had not been evaluated on the performance at least once every three years to ensure that they had the necessary knowledge and skills for the safe operation of the vehicle.
