29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  430 Island Pond Rd., Springfield, MA: On or about January 5, 2023, 2 employees were observed working on a residential structure engaged in masonry activities with unprotected sides and edges greater than 9 feet above the next lower level  and were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall from height hazards.  Mr. Home, Inc was previously cited for a violation of this occupational safety and health standard  29 CFR 1926.501(b)13, which was contained in OSHA inspection number 1355792, citation number 1, item number 1 and was affirmed as a final order on 3/22/2019, with respect to a workplace located at 118 Hopkins Pl., Longmeadow.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who were exposed to fall hazards:  430 Island Pond Rd., Springfield, MA: On or about January 5, 2023,  2 employees were exposed to fall hazards which are part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.  Mr. Home, Inc was previously cited for a violation of this occupational safety and health standard  29 CFR 1926.503 (a) 1, which was contained in OSHA inspection number 1355792, citation number 1, item number 2a and was affirmed as a final order on 3/22/2019, with respect to a workplace located at 118 Hopkins Pl., Longmeadow.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  430 Island Pond Rd., Springfield, MA: On or about January 5, 2023, 2 employees were observed using an aluminum extension ladder (with stabilizer bar) that only extended 1 foot past the upper landing surface (roof).  Mr. Home, Inc was previously cited for a violation of this occupational safety and health standard  29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1355792, citation number 1, item number 3 and was affirmed as a final order on 3/22/2019, with respect to a workplace located at 118 Hopkins Pl., Longmeadow.

29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder exceeded approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  430 Island Pond Rd., Springfield, MA: On or about January 5, 2023, the employer had not ensured that  2 employees using  a portable extension ladders to access an upper level  had been installed in manner in which the working angle did not exceed a ratio of 4:1 or one quarter the working length of the ladder.
