29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards: a) Jobsite at 241 Woodbine Rd, Downingtown, PA- On or about January 5, 2023, the employer did not provide a training program for each employee conducting roofing activities in the recognition and avoidance of fall hazards. Employees were working on the roof of a commercial building without the use of fall protection, thereby exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a) Jobsite at 241 Woodbine Rd, Downingtown, PA- On or about January 5, 2023, the employer did not ensure that the ladder used to access the upper landing surface of the roof was extended to at least 3 feet. The employee used an aluminum Werner ladder not extended at least 3 feet to access the upper landing surface of the roof to perform roof repairs, thereby exposing the employee to fall hazards of approximately 20 feet.
