One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. At the Trailer Repair Bay area of the facility, located at 2875 US 1 Highway in Saint Augustine, Florida 32086: On January 5, 2023, and at times prior, the employer exposed employees to amputation hazards, in that a Sears Craftsman brand. 15-inch Drill Press was not guarded as employees performed drilling operations on metal work.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. a. At the Trailer Repair Bay area of the facility, located at 2875 US 1 Highway in Saint Augustine, Florida 32086: On or about January 5, 2023, and at times prior, the employer exposed employees to struck-by and crushed-by hazards, in that the employer did not ensure that each Powered Industrial Truck (forklift) operator had been trained and evaluated as required by this paragraph (l), prior to the employees operation of the truck.
