29 CFR  1903.19(c)(1):Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.  On or about April 17, 2023, the employer failed to submit certification of corrective action for citations issued on February 23, 2023, with an abatement due date of March  23, 2023.

29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about January 5, 2023, Employees were exposed to fall hazards in excess of 18ft without the use of fall protection while working on a scaffold platform.

29 CFR  1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  On or about January 5, 2023, Employees engaged in decking operations were exposed to fall hazards while on a scaffold in excess of 18ft as the employer failed to provide training.
