Location: 60 Black Brook Rd. Bedford, NY 10506 a) On or about January 5th, 2023 raised on forklift : An employee fell while raised on a plywood pallet attached to the arms of a forklift while finishing metal edging on a residential residence roof approximately 8ft above the ground. The employee was not protected by a complete guardrail system, and the employee was not wearing any fall protection devices.
