29 CFR 1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a) On Jan. 6, 2023, employees were exposed to a fall hazard while working from the platform of a JLG 860SJ extensible boom lift, without being attached to the boom or basket, at a fall height of approx. 57 feet above ground level.

29 CFR 1926.502(d)(9): Lanyards and vertical lifelines shall have a minimum breaking strength of 5,000 pounds (22.2 kN). (a) On or about the 6-month period of July 6, 2022 - Jan. 6, 2023, the employer provided a personal fall arrest system (PFAS) to employees that contained a lanyard not rated for this application while working from the elevated platform of extensible boom lifts, in that the PFAS contained a cross arm strap in a 'choked' configuration for use as an extension lanyard between the harness attachment point on the wearer's back and a retractable lanyard snap hook.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. (a) On Jan. 6, 2023, employees performing ICF work at the interior roof structure of a residential building were exposed to fall hazards while standing on an elevated wooden support beam without use of a fall protection system at a fall height of approx. 11 feet to the next lower level.
