29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the evaluation specified in this paragraph (l): Jobsite: 5929 Greene St. Phila., PA 19144 - On or about January 6, 2023, the employer did not ensure that each powered industrial truck operator received an evaluation prior to operating powered industrial trucks, thereby exposing employees to struck-by hazards. Evaluation shall consist of a combination of formal instruction and practical training of the operator's performance in the workplace.
