On or about January 18, 2023, at an establishment located at 200 Woodland Drive in Random Lake, Wisconsin; woodworking employees with prescription lenses were exposed to flying particles when they were not provided eye protection that incorporated the prescription into its design, or wore eye protection over the prescription lenses.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  At an establishment located at 200 Woodland Drive in Random Lake, Wisconsin; employees were exposed to machine hazards associated with moving parts when they were engaged in servicing and maintenance activities on equipment such as, but not limited to, molders and profilers, belt sanders, and rip saws. The employer did not ensure that employees implemented energy control application steps or equivalent protective measure prior to allowing them to conduct such activities as cleaning, clearing jams, changing blades and belts, and other maintenance activities. The machines were not deenergized prior to employees engaging in the servicing and maintenance activities.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  At an establishment located at 200 Woodland Drive in Random Lake, Wisconsin:  a) On or about January 6, 2023, employees were exposed to entanglement hazards while operating a Delta drill press (S/N: 8810) with an unguarded chuck. The machine was not effectively guarded to prevent exposure to the entanglement hazard.  b) On or about December 31, 2022, to January 6, 2023, employees were exposed to pinch point and amputation hazards created by the HP 2000 Case Clamp machine (S/N: 1250) plungers and the wooden table being clamped. The machine was not effectively guarded to prevent exposure to these hazards.

Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  At an establishment located at 200 Woodland Drive in Random Lake, Wisconsin; the manufacturer-supplied sheet metal guards on the cutting heads of the Weinig CNC Molder #195 (Model: P 23; Machine No.: 123-1168) were not affixed to prevent employee exposure to laceration and amputation hazards created by the rotating blades.

29 CFR 1910.213(b)(7): Feeder attachments shall have the feed rolls or other moving parts so covered or guarded as to protect the operator from hazardous points.  At an establishment located at 200 Woodland Drive in Random Lake, Wisconsin:  a) On or about December 20, 2022, to January 6, 2023, the Sicotte Horizontal Boring Machine (Serial Number: 904; Model: J-20) did not have guarding to prevent employee access to the pinch point created by the clamping piston(s) and the wood stock, and the sheer points created by the carriage when the machine moved horizontally during the boring process. Employees were exposed to amputation hazards of the hands and fingers created by these pinch and sheer points during production.  b) On or about January 5, 2023, the Northtech Mortise Machine #197 (Model: MS-25A, S/N: 1743) was not guarded to prevent employee access to the pinch point created by the clamping piston and the wood stock. Employees were exposed to amputation hazards of the hands and fingers created by the pinch point during production.  c) On or about December 30, 2022, to January 2, 2023, the Onsrud Profiler (S/N: 7825) did not have guarding to prevent employee access to the pinch points created by the clamping pistons and the wood stock. Employees were exposed to amputation hazards of the hands and fingers created by these pinch points during production.  d) On or about January 9, 2023, to January 13, 2023, the Schelling Cut-to-size saw (Machine No.: 241.328) did not have guarding to prevent employee access to the pinch points created by the clamping pistons and the wood stock. Employees were exposed to amputation hazards of the hands and fingers created by these pinch points during production when clearing cut off pieces of panels from the cutting area.

29 CFR 1910.213(c)(1): Each circular hand-fed ripsaw shall be guarded by a hood which shall completely enclose that portion of the saw above the table and that portion of the saw above the material being cut. The hood and mounting shall be arranged so that the hood will automatically adjust itself to the thickness of and remain in contact with the material being cut but it shall not offer any considerable resistance to insertion of material to saw or to passage of the material being sawed. The hood shall be made of adequate strength to resist blows and strains incidental to reasonable operation, adjusting, and handling, and shall be so designed as to protect the operator from flying splinters and broken saw teeth. It shall be made of material that is soft enough so that it will be unlikely to cause tooth breakage. The hood shall be so mounted as to insure that its operation will be positive, reliable, and in true alignment with the saw; and the mounting shall be adequate in strength to resist any reasonable side thrust or other force tending to throw it out of line.   At an establishment located at 200 Woodland Drive in Random Lake, Wisconsin; the Delta table saw (S/N: 1322, Model 34888) was guarded by a hood which did not completely enclose that portion of the saw blade above the table and material being cut. The hood was shifted backward out of alignment with the saw, exposing the front portion of the blade.

29 CFR 1910.213(c)(2): Each hand-fed circular ripsaw shall be furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator. The spreader shall be made of hard tempered steel, or its equivalent, and shall be thinner than the saw kerf. It shall be of sufficient width to provide adequate stiffness or rigidity to resist any reasonable side thrust or blow tending to bend or throw it out of position. The spreader shall be attached so that it will remain in true alignment with the saw even when either the saw or table is tilted. The provision of a spreader in connection with grooving, dadoing, or rabbeting is not required. On the completion of such operations, the spreader shall be immediately replaced.  On or about January 6, 2023, at an establishment located at 200 Woodland Drive in Random Lake, Wisconsin; the Delta table saw (S/N: 1322, Model 34888) was not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator.

29 CFR 1910.213(m)(1): The cutting heads of each wood shaper, hand-fed panel raiser, or other similar machine not automatically fed, shall be enclosed with a cage or adjustable guard so designed as to keep the operator's hand away from the cutting edge. The diameter of circular shaper guards shall be not less than the greatest diameter of the cutter. In no case shall a warning device of leather or other material attached to the spindle be acceptable.  At an establishment located at 200 Woodland Drive in Random Lake, Wisconsin:  a) On or about December 30, 2022, to January 2, 2023, the cutting head on the Onsrud Profiler (S/N: 7825) was not guarded to keep the operator's hand away from the cutting edge. The employee was potentially exposed to laceration, entanglement, and amputation hazards created by the spinning blade while hand-feeding the wood stock into the blade for shaping.  b) On or about January 6, 2023, the Porter Cable router that was mounted underneath a router table was not guarded to keep the operator's hand away from the cutting edge as the employee hand-fed wood stock into the cutting area. The employee was exposed to laceration, entanglement, and amputation hazards created by the spinning blade as the operator's hand passed over the blade while pushing the wood stock over the cutting head.
