29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use (Construction Reference 29CFR 1926.103):   a) 309 Akers Ln, Whitefish, MT:  On or about January 4, 2023, and at times prior, employees were exposed to chemical hazards while wearing half mask elastomeric respirators with multiple types of filters and applying two part spray foam insulation at worksites without a respiratory protection program being implemented prior to the employees using the respirators.  b)309 Akers Ln, Whitefish, MT:  On or about January 4, 2023, and at times prior, employees were exposed to chemical hazards while wearing half mask elastomeric respirators with multiple types of filters and applying two part spray foam insulation  without prior precautions or work place safety inspections being conducted to determine which type and model respirators and filters would be required.   c)309 Akers Ln, Whitefish, MT:  On or about January 4, 2023, and at times prior, employees were exposed to chemical hazards while wearing half mask elastomeric respirators with multiple types of filters and applying two part spray foam insulation  without having medical evaluations conducted prior to the use of respirators by the employees.    d)309 Akers Ln, Whitefish, MT:  On or about January 4, 2023, and at times prior, employees were exposed to chemical hazards while wearing half mask elastomeric respirators with multiple types of filters and applying two part spray foam insulation  without previously being fit tested by qualified personnel.   e)309 Akers Ln, Whitefish, MT:  On or about January 4, 2023, and at times prior, employees were exposed to chemical hazards while wearing half mask elastomeric respirators with multiple types of filters and applying two part spray foam insulation while using respirators that had not been properly inspected, cleaned, stored, or sanitized before the use of the respirators by the employees.  f) 309 Akers Ln, Whitefish, MT:  On or about January 4, 2023, and at times prior, employees were exposed to chemical hazards while wearing half mask elastomeric respirators with multiple types of filters and applying two part spray foam insulation on the jobsite without having adequate training on respirator use.
