29 CFR  1926.451(a)(1):Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it:.  At a residential construction site located at 33 W Shore Drive, Lake Lotawana, Missouri.  Employees were exposed to a fall hazard as they performed work from a man made scaffold platform which was not capable of supporting  the weight of the workers and equipment using scaffold. The scaffold collapsed while workers were installing a metal beam weighing approximately 300 pounds.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.   At a residential worksite located at 33 W Shore Drive, Lake Lotawana, Missouri.  Employees were performing residential construction operations, adding on to an existing structure, at an elevated height on a of approximately 9.5 feet without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.
