Pendergraft Construction did not ensure electrical equipment inspections were occurring before use, leading to the use of equipment with recognizable hazards.  1. Pink extension cord labeled Pendergraft Construction with fraying and nicks 2. Circular saw attached to pink extension cord with fraying and nicks and section covered with electrical tape 3. Flexible cord through ceiling hole 4. Missing ground pin on pink extension cord

Employees of Pendergraft Construction were operating a circular saw with a pink extension cord labeled Pendergraft Construction missing a ground pin.

At the worksite located at 1924 East 20th Street Joplin, MO 64804: Employees were exposed to fall hazards. Employees were working at heights on a roof around a hole more than six feet above lower levels without fall protection or a guardrail system in place.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the worksite located at 1924 East 20th Street Joplin, MO 64804: Employees were exposed to fall hazards. The unprotected sides and edges of the elevated work area were not provided with guardrails or any other means of fall protection. The employees were exposed to a fall hazard of 9 feet 11 inches.   Pendergraft Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1387432, citation number 1, item number 3 and was affirmed as a final order on 05/28/2019, with respect to a workplace located at South Ellis Street and Fountain Road, Webb City, Missouri 64870.

Employees were accessing a working surface by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.    Pendergraft Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1387432, citation number 1, item number 4 and was affirmed as a final order on 05/28/2019, with respect to a workplace located at South Ellis Street and Fountain Road, Webb City, Missouri 64870.
