29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a. 4 Brothers Roofing & Renovations LLC located at 6212 Kalleva St., Lot 100, Westerville, OH 43081: the employer failed to initiate and maintain safety/health programs such as but not limited to fall protection/prevention prior to employees conducting residential construction activities. The following condition exposed employees to falls of over 20 feet.

29 CFR  1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   a. 4 Brothers Roofing & Renovations LLC located at 6212 Kalleva St., Lot 100, Westerville, OH 43081: On or about January 9, 2023,  the employer did not ensure employees exposed to fall hazards were provided with adequate anchorages while performing roofing work using a personal fall arrest system. Roofers were installing shingles on a 10/12 pitch roof that varied from 20-30 feet. The roofers were wearing full body safety harnesses with lines tied to the roof trusses/nail plates as anchor points. The employees were not utilizing compliant fall protection which exposed them to falls over 20 feet.

29 CFR  1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   a.  4 Brothers Roofing & Renovations LLC located at 6212 Kalleva St., Lot 100, Westerville, OH 43081: On or about January 9, 2023, the employer did not provide a training program for each employee exposed to fall hazards to ensure each employee to recognized the hazards of falling and the procedures to be followed in order to minimize these hazards.
