29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): Employee engaged in maintenance activities at 1703 N Harrison O'Neill, Nebraska on or about October 14, 2022, were exposed to amputation hazards in that the employees were not provided with an energy control program by the employer that met the requirements of 1910.147, including but not limited to; a) Authorized employees servicing equipment, b) Non-authorized employees whose work operations may be in a area where energy control procedures are being utilized and; c) Specific energy control procedure for the Super Biochopper.
