Walnut Street (Dover Pools) in Milford, DE 19963: Employees performed roofing and gutter work on a commercial structure at heights of approximately 12 feet above the ground level without any means of fall protection.

Walnut St., Milford, DE 19963: Employees used ladders that did not extend at least three feet above the upper landing to access to the roof. The ladder had one rung exposed above the roof to gain access.
