On a roof of a new multifamily residential, at a temporary worksite located at 5255 Race Track Road, in Saint Johns, Florida: On or about January 10, 2023, the controlling employer La costa framing LLC, failed to ensure that its sub-contractor JADAC LLC,��protected its employees from fall hazards ranging from 6 to 42-foot, in that fall protection was not utilized by employees while performing framing work to install trusses.
