29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about January 10, 2023, at 320 E Main Street in Hortonville, Wisconsin, employees were exposed to a fall hazard while using a Skyjack scissor lift to access/egress a roof surface at greater than 14 feet above surface level without connecting the gate chain to complete the guardrail system and when employees climbed over the guard rails to access the roof.
