South Side Production Area - On or about January 10, 2023, the fire exit door did not provide free egress/escape from the building in that the door was locked with a padlock, thereby exposing employees to the hazards associated with fires and other emergencies. b. 29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): a. Production Area - On or about January 10, 2023, the employer did not ensure that all point of operation guards on machinery, including Minster Piece Maker Press #15 and #17 and Oak Punch Press #26 and #27, were adequately designed to prevent employees form having their hands or fingers in the danger zone. b. 235 N. Artesian Avenue, Chicago, IL - Production Area - On or about January 10, 2023, the employer did not ensure that all point of operation guards on machinery, including Minster Piece Maker #2 and #6, and Oak Press Punch #9 were adequately designed to prevent employees from having their hands or fingers in the danger zone. Lamination Specialties was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.217(c)(2((i)(a), which was contained in OSHA inspection number 1295467, citation number 1, item number 2 and was affirmed as a final order on September 28, 2018, with respect to a workplace located at 2254 W. Fulton, Chicago, IL 60612.
