Building 13026 Maintenance Bay: On and/or before January 10, 2023, employee(s) used a damaged step ladder to access a lifted vehicles elevated position that was on an alignment rack to perform automotive work. The step ladder's second step was crushed by the alignment rack and was not tagged, repaired, or removed from service. This condition potentially exposed employee(s) to mechanical failure, fall, and/or trip hazard(s).

29 CFR  1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors.  Building 13027, Retail store, 2nd floor, storage room for tires: On and/or before January 10, 2023, the far north isle between the rack and HVAC system, the condensation line in close proximity to the electrical cord that powers the emergency light were running through the floor was plugged into the wall outlet. Employee(s) would retrieve and stock tire(s) daily around this location. This condition potentially exposed employee(s) to electrical and/or fire hazard(s).

29 CFR  1910.36(g)(2): Exit access were not at least 28 inches (71.1 cm) wide at all points. Aisles were not of sufficient width to allow the safe evacuation of all the people who work on the floor served by the aisle.  Building 13027, Retail store, 2nd floor, Storage room for tires: The far north isle was measured to be 22.5 inches in width. Employees were exposed to a narrow escape route and fire hazard.  Firestone Complete Auto Care was previously cited for a violation of this occupational safety health standard or its equivalent standard 29 CFR 1910.36(g)(2), which was contained in OSHA inspection number 1435913, citation number 1, item number 4, and was affirmed as a final order on March 24, 2021, with respect to a workplace located at 13027 Vandergrift Blvd Camp Pendleton, CA 92055.

The fire extinguisher was located on the wall across the north aisle behind a stack of tires and was not made readily accessible. This condition potentially exposed employee(s) to fire hazard(s).
