29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Employees engaged in material movement using the Komatsu Forklift, SN: 468871A, were exposed to struck-by and tip-over hazards when using modified forks to move material. The forks were modified without the prior approval by the manufacturer.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; Employees engaged in operations within the seed shed were exposed to fire hazards in that three oxygen and three acetylene compressed cylinders were stored directly next to and within the same storage area. Compressed oxygen cylinders and acetylene cylinders were stored directly next to each other with a separation of 2 inches.
